Assistic Care Services

Companion services provide a non-medical assistance and supervision to support a person’s goals, desires and needs, as identified in the person’s Individual Support Plan(ISP) or Plan of care to promote the person’s safety, self independence, community integration, and/or retirement.

This service enables people to live in the community in a family-type setting that will support them to achieve their goals, participate in community life and activities, maintain their health, and retain or improve skills that are important to them.

Assistic Care Services Inc. provides In-Home Support Services to persons living in their own home or living in their family member’s home. In-Home Support services entails services that provide habilitative support, personal care, assistance with activities of daily living and other support services to the person in their home.

Quality transitional services assisting individuals smoothly navigate structural, domestic or residential moves, ensuring continuity of standard support, health access, and immediate integration.

Residential Habilitation Services is provided in a most integrated home setting, for persons with Intellectual and Developmental Disabilities, serving four(4) to six(6) persons. Services are provided in accordance with each person’s approved Individual Support Plan (ISP) and Plan of Care.

Supported Living Services are provided to persons who require specified formal and informal supports in a home and have an assessed need for assistance with acquisition, retention, or improvement of skills related to activities of daily living, personal care and nursing.

Respite care services is provided for short-term or temporary relief for parents, caregiver, custodians or those who are caring for family members enrolled in the Home Community Based Services Waiver. Respite care will ensure that persons will continue to receive services and have access to community activities.
